Back in the early 1990's the Internet was controlled by a handful of US academic institutions: they ran the DNS servers, they ran the core routers etc. everyone else just followed.

As I say there was a window, now long closed, that was recognised at the time, when people called on the IPv6 working group to deliver something usable quickly.

TCP/IP had effectively won in 1989, it just took time for people to migrate off DECnet, SNA, OSLan, XNS, etc.
