I'm not sure the issue was insufficient testing - I mean the testing that was carried out was certainly insufficient but the tests were also significantly scaled back until the system would pass. If you are going to move the goal posts to ensure your tests are passed regardless of the outcome AND you don't intend to address any of the findings, I'm not sure any testing will produce a worthwhile outcome.
The report also appears to show a lot of people who knew the project was in trouble and tried to get assurances from other parties that "all necessary checks had been carried out" to cover their asses rather than trying to address the issues.
The conclusion should probably have said "the board gave the project two extra years to complete the migration and then said it won't get any more ready than it is, fingers crossed"
