The biggest catalyst for IPv6 has been mobile devices. Mobile networks are already doing carrier-grade NAT for IPv4, with all the ugliness that entails. (It seems OK until you have tethering that creates another NAT behind the NAT, then things get tricky.) IPv6 actually works better when it's available, and many carriers have deployed it. It's just not very noticeable because few people look at the network configuration of their phone. My T-Mobile phone usually gets a globally-routeable IPv6 address and an RFC1918 IPv4 one.
