Erm, hasn't everyone unplugged a secondary PSU that is on whilst taking out a failed PSU?
In my defence I had a PSU half out when the cable for the second fell out of the cable management, I went to put it back , but pulled the cable out followed by the PSU dropping on my foot and lots of swearing.
