Way back in the day, I was working Main Frame ops for a large global corp.

After a newly changed program failed on system start for the day, the on call manager advised all would be okay. 4 hours later he called to ask what his advice had been, after realising his mistake much hilarity ensued as the whole system had to be restored and taken offline for the rest of the day.
