I've run into something similar. Our username convention is last name first initial. We made an exception for a kid named T. Shi when he started school though.
I tried to change my name at school to Ronald Oot, since I had access to the student information system. My boss, the system administrator, must have seen this before because the account creation job that ran nightly didn't create my new account. I was rather disappointed, and had a previously unscheduled meeting with him the next day...
I have worked at two small enterprise jobs where the scheme <firstname><initial-of-surname> was used for usernames and/or email addresses. It was only briefly policy in either case; in the former we had second thoughts about giving someone who came in at managerial level "christ@....".
